Member Without Card The form you use at events for Members without a card to sign in.
Non Member Indemnity The form you use at events for Non Members to sign in.
Minor Indemnity The form you use at events for Minor Members and Non Members to sign in.
Site Indemnity The form you use at events for Members with cards to sign in.
Transfer of Guardianship Form This is intended for use when a parent or legal guardian
will not be the guardian for the purposes of SCA activities.
I am going to an event, what forms do I need?
If you are going along to a SCA event, you will need to fill out some paperwork at
the site, and may need to bring some along with you. All of this is for insurance purposes.
If you are a minor (under 18) and your parents or legal guardian is not coming with
you, you will need to bring along a filled out Transfer of Guardianship Form.
In this form, your parents/legal guardian will appoint an adult who will be attending the event
to act as your legal guardian and fill out any further forms on your behalf. If this is your
first event and you and your parents do not know anyone who will be attending yet,
feel free to call the Event Steward (the person running the event)
or one of our members on the Group Contacts page and we will try
to
organise a guardian for you.
