Where To Find The Right Forms

You can find the appropriate form on the Marshallate forms webpage. If the links are incorrect, please let us know.

    Which Form To Use

Adult Authorisation form (Australia & New Zealand)
    - Use this form for all adult authorisations.

    Minor Authorisation Form (Australia)
    - Use this form for authorising a minor living in Australia. Include all the other required paperwork as listed on the Marshallate forms webpage.

    Minor Authorisation Form (New Zealand)
    - Use this form for authorising a minor living in New Zealand. Include all the other required paperwork as listed on the Marshallate forms webpage.

Completing The Forms & Making It Legal

Complete your details as required, and have your authorising marshal do the same for their section. Make a copy of the form and keep it with you. This copy is valid as proof of authorisation for 3 months from the date of the signature of the authorising marshal.

Completed forms must be sent to the appropriate Lists address for your country, as detailed on the bottom of the form. You must include a stamped self-addressed envelope, so that your card may be returned to you.
